Hovedutfordringer i informasjonsflyten under covid-19-pandemien : en casestudie av Akershus universitetssykehus

Abstract

Covid-19-pandemien medførte mange innleggelser av smittede pasienter som trengte medisinsk hjelp. Kapasiteten til sykehusene i Norge ble utfordret, noe som førte til store omrokkeringer på sykehusene for å ha ressurser til å behandle covid-19-pasientene. For å håndtere dette ble det en økning av samarbeid på tvers av enheter, der hele enheter praktisk talt ble slått sammen, og mange ansatte ble omplassert til andre enheter på sykehuset, kombinert med at beredskapsledelsen ble etablert som en ny beslutningstaker. Disse faktorene førte til en stor organisasjonsendring, og dette sammen med en pandemi, skapte utfordringer for informasjonsflyten.

Bakgrunnen for denne oppgaven er utførelsen av en casestudie av informasjonsflyten ved Akershus universitetssykehus under begynnelsen av covid-19-pandemien. Det ble utført 22 dybdeintervjuer med ansatte i forskjellige enheter som har hatt viktige roller i håndteringen av covid-19-pasientene på Akershus universitetssykehus. Intervjuene kombinert med dokument- og litteraturstudien er utgangspunktet for resultatene og diskusjonen. Informasjonsflyten presenteres ved hjelp av et kart, og på bakgrunn av kartleggingen blir flaskehalser presentert.

Resultatene viser at covid-19-pandemien har medbrakt flere utfordringer for informasjonsflyten ved Akershus universitetssykehus, der våre hovedfunn er: begrenset kommunikasjon mellom beslutningstakerne, flere nye parallelle informasjonslinjer skapte problemer, begrenset informasjonsflyt for ansatte utenfor sykehuset, informasjonsflyten oppover er begrenset og mangel på informasjon medførte konsekvenser ved informasjonsflyten. Avslutningsvis konkluderer vi med at metoden brukt for å analysere informasjonsflyten, ved hjelp av Lean metodikken, fungerte godt, og vi tror Lean kan være et nyttig verktøy for fremtidige analyser av informasjonsflyt.

The COVID-19 pandemic resulted in many hospitalizations of infected patients in need of medical attention. The capacity of the hospitals in Norway was challenged, which led to major reshuffles of staff in the hospitals to have the resources to treat the COVID-19 patients. To deal with this, there was an increase in collaboration across units, where entire units were merged, and many employees were relocated to new units at the hospital, combined with the establishment of emergency management as a new decision-maker. These factors led to a major organizational change, and this together with a pandemic created challenges to the flow of information. The background for this thesis is the execution of a case study of the information flow at Akershus University Hospital during the beginning of the COVID-19 pandemic. 22 in-depth interviews were conducted with employees in various units who have played important roles in the management of COVID-19 patients at Akershus University Hospital. The interviews combined with the document and literature study are the foundation for the results and the discussion. The information flow is presented through a map, and based on the mapping, bottlenecks are presented. The results show that the COVID-19 pandemic has brought several challenges to the information flow to Akershus University Hospital, where our main findings are: limited communication between decision-makers, several new parallel information lines created problems, limited information flow for employees outside the hospital, information flow upwards is limited and lack of information brought consequences to the flow of information. We conclude that our method used to analyze the information flow, using the lean methodology, worked well. We believe lean methodology can be a useful tool for future analyzes of information flow.
