Folkehelse og planlegging : en casestudie av integrering av folkehelse i kommunal planlegging

Sammendrag
Bakgrunn: Temaet for denne studien er folkehelse og planlegging. Planlegging er et viktig verktøy for å nå nasjonale mål for folkehelsepolitikken. Målene er å skape et helsefremmende samfunn som reduserer sosiale ulikheter i helse og øker antall leveår med god helse og trivsel. Forskning viser ulikheter i hvor langt kommuner har kommet i å ta i bruk dette verktøyet i folkehelsearbeidet.
Formål: Formålet med studien har vært å få kunnskap om hvordan det brede folkehelsebegrepet er forstått og integrert i plan, og på hvilken måte kommunale aktører vurderer at etter- og videreutdanningsprogrammet «Helse og omsorg i plan» (Helomplan) har påvirket denne integreringen.
Metode: Casestudie er valgt som design. Metodene som er brukt for å samle inn data, er analyse av plandokumenter og kvalitative intervjuer med to ansatte i en kommune.
Hovedfunn: Det synes å være en bred forståelse av folkehelse og at folkehelse er et tverrsektorielt ansvar. Likevel ser det ut til å være utfordringer knyttet til å få hele kommunen til å tenke helse når de planlegger samt å få forståelse av roller og oppgaver i folkehelsearbeidet. Folkehelse er et tema som tydelig vektlegges og er integrert i kommunens plandokumenter. Forebygging, tidlig innsats, helsefremming og sosiale ulikheter i helse er adressert i planene. Kommunen har folkehelsekoordinator som er strategisk plassert i rådmannens stab, og det er utarbeidet oversikt over helsetilstand og påvirkningsfaktorer for helse. Det imidlertid uklart hvorvidt oversiktsdokumentet blir brukt ved fastsettelse av mål i planer, slik det systematiske folkehelsearbeidet forutsetter. Det er etablert arenaer for tverrsektorielt samarbeid i kommunen. Studien frembringer ikke kunnskap om på hvilken måte kommunale aktører vurderer at Helomplan har påvirket integreringen av folkehelse i planlegging. Background: The theme of this study is public health and planning. Planning is an important
tool in reaching the national public health goals. The goals are to develop a health promoting
society that equalize social health differences and increase the number of years lived with
good health and well-being. Research shows differences in how far municipalities have
reached in making active use of this tool in public health work.
Objective: The objective of this study is to generate knowledge about how public health is
understood and integrated into municipal planning and in which way municipal actors
consider the educational program “Helse og omsorg i plan” (Helomplan) has influenced the
integration.
Method: A case study design is chosen. The methods used for collecting data are analysis of
planning documents and qualitative interviews with two employees in a municipality.
Main findings: It seems to be a broad understanding of public health and that public health is
a cross sectoral responsibility. Nevertheless there seems to be challenges in getting the whole
municipal organization to include health in their thinking when planning, as well as to get
understanding of the roles and responsibilities in the public health work. Public health is a
theme that is emphasized and integrated in the municipal plans. Prevention, early intervention,
health promotion and social inequities in health are addressed in the plans. The municipality
has a public health coordinator strategically placed in the staff of the deputy mayor. They
have made a document showing the state of health and factors influencing health. However, it
is unclear whether this document is used when goals are set in plans, as the systematic public
health-work requires. Arenas are established in the municipal organization for cooperation
across sectors. The study did not bring forth knowledge of in which way Helomplan has
influenced the integration of public health in municipal planning.
